Purpose
The purpose of this Hazard Identification and Risk Assessment (HIRA) is to identify and assess the hazards in the workplace and to develop controls to mitigate the risks associated with those hazards.
Scope
This HIRA applies to all employees, contractors, and visitors to the workplace.
Responsibilities
- Management: Management is responsible for ensuring that this HIRA is conducted and that the controls identified in the HIRA are implemented.
- Employees: Employees are responsible for participating in the HIRA process and for reporting any hazards or unsafe conditions to their supervisor.
Procedure
The HIRA will be conducted using the following steps:
- Identify the hazards: The first step is to identify all of the potential hazards in the workplace. This can be done through a variety of methods, such as inspections, job hazard analyses, and employee surveys.
- Assess the risks: Once the hazards have been identified, the next step is to assess the risks associated with each hazard. This involves considering the likelihood of the hazard occurring and the severity of the potential consequences.
- Develop controls: Once the risks have been assessed, the next step is to develop controls to mitigate those risks. Controls can be engineering controls, administrative controls, or personal protective equipment (PPE).
- Implement the controls: Once the controls have been developed, they need to be implemented in the workplace.
- Monitor and evaluate the controls: The effectiveness of the controls needs to be monitored and evaluated on a regular basis.
Example
The following is an example of a HIRA for a task that involves using a power saw:
Hazard | Risk | Control |
---|---|---|
Electric shock | Medium | Use a properly grounded power saw. |
Cuts and lacerations | High | Wear appropriate safety gear, such as safety glasses, gloves, and long pants. |
Amputations | High | Be careful when operating the power saw and keep your hands away from the saw blade. |
Additional Considerations
The HIRA process should be ongoing and should be updated on a regular basis to reflect changes in the workplace and new information about hazards. It is important to involve employees in the HIRA process to ensure that all hazards are identified and that the controls are effective.
